Forward uses a different payment model than other healthcare providers. Instead of charging for individual services, patients (or “members”) pay a monthly subscription fee covering most services. As of 2021, you’ll pay $149 per month with no copays or additional bills. Forward requires a minimum commitment of 6 months; however, you can pay $1,639 for a one-off annual membership.
